This patient came to us after being told his only option for his LL7 was extraction. The tooth was heavily filled with a poorly fitted aesthetic post in the distal. A CBCT confirmed the presence of a missed ML canal and a possible apical fracture in the distal canal. We decided to treat this over multiple visits and re-treat just the mesial canals to see if his symptoms decreased. We did not want to risk instrumenting the distal if just selective treatment was successful. After RCT 1, his buccal swelling and pain had resolved entirely, and we just retreated his mesial canals. He later went on to replace the space with dental implants with Dr Marques.
